Standardizing physical exam packet – Rich Mummey is working on a standardized test result form, a standardized contract for services, a consent for release form and a procedure for handing physical results. The Chiefs are working on a statement of works. All should be ready by the November meeting. A question was asked whether the consent form would have to be signed prior to taking the physical. The answer is no, the consent form needs to be signed only if the information from the physical exam is to be released to someone else. A statement will be included in the contract for services that a copy of the results will be sent to the individual being given the physical. Disposition of Issue: Ongoing.
3. Military Leave Usage Tracking – Employees and/or managers would like some assistance tracking military leave. The question was asked whether military leave could be put on pay stubs. To get the balance put on pay stubs would take a computer program change at DAS and an approval from the Auditor of State. It was decided that the following methods will be used to determine military leave balances:
1) Employees check their pay stubs for hours used against the 172 hours given each January 1;
2) Employees ask their supervisors to let them see their leave slips so they can see how much time they’ve taken;
3) As a last resort, call Karen in SHRD.
Ultimately tracking leave usage is the responsibility of the employee. Disposition of Issue: Closed.
4. Honor Guard – A draft Honor Guard program was distributed for review and comment at the next meeting. Funding issues will have to be addressed. It was suggested that everyone read the FOMA Chapter 36 before the next meeting and review the draft program prior to the next meeting and bring back any questions or concerns. Disposition of issue: Ongoing.
5. Physical Ability Testing – A draft program was handed out. Discussion was held concerning the tasking of the committee. It was decided that the committee was to come up with a workable test, so the first page of the draft was tabled. Chief Matulja mentioned that he witnessed the test at his base and considers it to be an impressive, very workable test. Changes to be made to the test:
Page 2 Item 5 change 5-inch hose to 3-inch hose.
Page 3 Tasks 2 and 3 change 12 or 14-foot ladder to 14-foot ladder to make consistent.
Page 4 Task 6 change 5 inch hose to 3 inch hose
Page 4 Task 7, the committee will reword the task to change all usage to the Kaiser sled instead of having an option.
Page 4 Task 8 change the 4th sentence after the () is permitted, to is required.
Questions raised: Is there any time limit? No, but the test must be continuous.
Is there a rest period time limit? It was suggested to have a 2 minute bank of time for rest between tasks during the test.
Is there a pass/fail? The only fail is if the employee is unable to complete all 10 tasks in a continuous manner.
When will the test be in place? Minor edits can be made without the whole committee prior to the next meeting. Once that is done, the test can be used. The committee will have to determine what happens if a person passes and what happens if a person does not pass.
If someone calls in sick, can they make up the test? Yes, if off for a valid reason.
The ten tasks were approved with the changes mentioned above. A set of instructions will be drafted. Disposition of issue: Ongoing.
